Who needs the DA Form 3739?
The DA-3739 is the United States Department of the Army form that is officially called the Application for Compassionate Action. It is supposed to be used by a military service member if they believe they are eligible for a compassionate action, like reassignment, deferment, or deletion of orders.
What is the Application for Compassionate Action Form for?
The DA form 3739 is used to assist the service member in figuring out if he is eligible for a compassionate action according to USC Title 5, Section 301 due to reasons listed on the form.
When is the Form DA-37339 due?
The form does have an expiration or due date and can be filed when the need has arisen.
Is the DA 3739 Application Form accompanied by any other forms?
In case the soldier filing the Compassionate Action Application form is on leave, DD ALV — Date Departed for Authorized Leave, or in attaché status, they are obliged to attach Form DA-31 or the order itself to the filled out Application.
If the military member is requesting the compassionate action due to illness or injury, the relevant statement signed by the attending physician must be furnished as well.
How to fill out the DA Application Compassionate Action 3739 form?
To improve the chances of the application being approved, the soldier is required to provide all the relevant information. The items to cover include service information, marital status and data about family members (spouse and children, parents, siblings). It is also mandatory to indicate if the soldier has previously applied for compassionate actions, the reasons for the application and how the soldier has attempted to improve the situation.
Then, the soldier must authorize the filled out DA-3739 form by adding a signature and date. And finally, the commander or the authorized representative need to sign the document as well.
